Conflicts of Interest or Commitment disclosure form due October 9

Reminder: As part of Wake Forest’s commitment to ethics, integrity, and lawful behavior, we require exempt staff and full-time University faculty to complete an electronic Conflicts of Interest (COI) or Commitment disclosure form.

An email was sent to applicable faculty and staff on September 18 with a link to complete your electronic COI disclosure. The deadline to complete your electronic COI disclosure is October 9.

If you have not already completed your COI disclosure, please do that promptly. If you expected to receive a disclosure form but did not, please check your spam, junk, social, and promotions folders in case it landed there. If you still do not see the email, contact Compliance Services at COI@wfu.edu.
